    • We all probably know about Ely Lilly's Antibody RX - Regenraon - and that it got EUA about 2 weeks ago. I thought great, another tool in the tool shed to treat COIVD patients. Well, it turns out this stuff works but Lilly says it's not proven to prevent deaths. They initially studied that exact effect and, the results were unimpressive - you probably heard the press report that BAD NEWS on Regeneron maybe a week or so ago.

      What they didn't tell you because they don't fucking know and should just STFU is that Lilly redid their Phased Trial to see how well it kept at risk COVID + patients out of EDs and then being placed in hospitals. It does a good job at that - not gangbusters but good enough to make a difference in big hospital systems that are worried about getting maxed out.

      Here's the scenario: A 70 year old with COPD walks into the ED complaining of fatigue and that he'd probably been exposed to COVID at work. His temp is 102, his BP is WNL, his O2 sats are 97%. He gets a RAPID RT/PCR saliva tests and it's positive. This is a pretty high risk patient. Do you admit him for Obs assuming he's going to deteriorate rapidly? Not if you go to the Mayo Clinic ED in Minneapolis. They, along with several other big hospital systems nation wide under the auspices and funding of the HHS, are setting up trials of specialty out-patient infusion clinics designed specifically for patients like this. There are plenty of them ..... heard of protect the vulnerable approach and the complaining that this isn't happing from the media? Well, it is and it is national, federally funded and decentralized (like it should be) to big hospitals that have smart employees that can do this right and report up the chain on how it's working. So, the idiot media can kindly STFU Just a reminder that you won't hear any good news on the pandemic only the horrible bad news.

      This is really good news. A drug that is likely to keep ambulatory COVID +, at risk patients and even non-ambulatory but otherwise healthy nursing home patients out of the ICU is a huge deal right now. HUGE. An interesting side story is that there are initially 300K doses that were produced during Phase III Trials and around 1B doses could be produced over the next 12 months, Lilly says, if demand requires it. So, gasp, it's being rationed and there are protocols to determine who gets it and who doesn't. That this has become public knowledge is also a huge deal. That's becasue, the press will be all over this, not understanding why or presenting arguments that help the public understand why it's being rationed, but instead bashing doctors and hospitals for denying an 80 year old with end stage pancreatic CA this treatment.

      I just don't understand why Talent values the press and their reporting. It's just confusing to me.
      Mission to CFB's National Championship accomplished. But the shine on the NC Trophy is embarrassingly wearing off. It's M B-Ball ..... or hockey or volley ball or name your college sport favorite time ...... until next year.

                  Careful there. When you hit the last 10 years of your career, you will be at the apex of your salary and also at your highest tax bracket. Typically, your income is reduced when you enter retirement and so is your tax bracket. Weighting your tax-deferred vs post tax is something you should review every 3-5 years. I've rebalanced probably every 5-6 years on average.
