      • I heard a number today that surprised me. Medicaid funding contains $ 7,000.00 per person funding. So, a family of four would have $ 28,000 to purchase insurance if the funds were sent directly to those who qualify.

            I've no idea how anyone can think the Trump Org. and the White House have a firm separation between them at this point

                      The problem is, neither side is addressing the problem, they just want to keep paying for the symptoms. They argue who gets what subsidy, how insurance companies can adjust their rates, who gets contractual bennies or reductions, how doctors get paid, etc. Everybody is yapping about payment. NO one is talking about cost. Perhaps the pharmacutical and insurance cartels should be examined. Look at the amount of money that is spent on lobbying. If you bring down the cost, everybody pays less. Duh.

                      And ban prescription drug commercials like you ban cigarette ads. I'm sick of the absolute deluge of limp dick, itchy twat, depression drugs ads flooding in. If you can spend tens of millions of ad dollars on drugs that affect .01% of the population, you can lower your $%$% prices.

                        I'd agree with the author's central premise which in short he argues for the need to take politics out of the equation in determining a system for HC delivery in the US. That's not going to happen. We'll have to deal with polarizing positions in formulating HC legislation.

                        And, just to be clear, Medicaid, in particular, is a single payor system efficient in both delivering care and controlling costs. Most of this is due to the kind of competition introduced by the Center for Medicare and Medicaid Services.

                        Lost in all the bullshit and misinformation surrounding the Healthcare debate is the reality that hybrid systems, like Medicare and Medicaid, work. Yet, both of these programs are routinely cast as examples of government waste, fraud and abuse. What this characterization actually is an example of is say it loud enough and long enough it becomes an unassailable fact.

                        When I we see the HC debate as having to choose between a single payor system and one that we have now that is essentially defined by the ACA, as the author seems to imply, we miss a third option: one that is a hybrid like Medicaid actually is. I don't think that is well understood by those offering opinions on how to fix it. Some Medicaid facts:

                        Recent proposals at the federal and state levels to radically restructure Medicaid raise questions about how the program works and its current role for states, beneficiaries, and health care...
                        Mission to CFB's National Championship accomplished. But the shine on the NC Trophy is embarrassingly wearing off. It's M B-Ball ..... or hockey or volley ball or name your college sport favorite time ...... until next year.

                          Jeff... Medicaid is not this great answer. Several states are going broke due to Medicaid... they can't fund the obligations they have...

                          Can you explain why you think Medicaid is this third option? What about it makes it something different. I don't see it different than I see the Medicare program.. just who it targets and where it is funded. So curious as to why you see it as a unifier third option.

It's the concept of a federally structured single payor HC delivery system with market forces at play as regulating forces. l'll try to provide additional detail but Ive provided some of that in several previous posts. I'll go back and point to these.

                            • Jon, you nailed it! Drug companies I suspect spend more money on advertising than on research. We are the most over medicated people in the world as a result. And our health outcomes lag behind almost all the advanced societies.

                              And our private insurance companies have nowhere near the efficiency that single payer government programs have. Look at our VA as a prime example. Or compare our private insurance system to health care programs in Canada or Western Europe.

                              Oh I forgot! Capitalism is the most efficient system for health care, and everything else, including private education (ask Betsy DeVos), and our for-profit prison systems!

                              As Reagan said, government is not the solution, government is the problem. Just ask our current administration, or the 1% where expense in health care is little more than an annoyance. Meanwhile the cost of healthcare for the masses is soaring out of control.

                              We started down this path after WWII while the rest of the developed world gravitated to some form of single payer government funded system of health care. When I was in practice the vast majority of my colleagues were ardent supporters of the fee for service privatized model but I notice this is now changing as more docs now opt for fewer hours and salaried practices. I think most have become disillusioned with our traditional market based system. I guess you could call me a socialist! And I think back to my younger days when I was an ardent supporter of Nixon, or when I had a Goldwater bumper sticker back in 1964!

                              • I actually don't understand the need for insurance. Let health systems pool people together or employers. Insurance performed tasks computers and technology now allow others to do as or almost as efficiently.
